If you intend to certify cultivated mushrooms, please download and complete the Mushroom Cultivation Supplement. (Note: If you are ONLY certifying cultivated mushrooms, you do not need to complete a Farm Plan, just the Mushroom Cultivation Supplement). Please reference the Guidelines for Organic Mushroom Production here.

Organic Handling Plan (OHP)

There are instances when a farm operation will need to complete an Organic Handling Plan (OHP); these include on-farm poultry slaughtering and other value-added products (e.g., bottling milk or fermenting vegetables). For simplified on-farm processing please complete the On-Farm Processing form. You must also complete either a Multi-Ingredient or Single-Ingredient Processed Product form for each product you intend to certify. If you are uncertain whether to complete the OHP or the On-Farm form, please contact our office.

Hemp

With the passage of the 2018 Farm Bill, hemp is no longer a controlled substance and is now a legal agricultural crop in the USA. To apply for hemp certification under the USDA National Organic Program, please refer to the required crops forms on this page. In addition to these forms you will need to complete the supplemental forms on our hemp page.

Please complete your application online, or you may mail your paper application to: MOFGA Certification, PO Box 170, Unity, ME 04988. Fees and a completed fee sheet must accompany your application.

You can pay through our secure online portal, or fees can be mailed in (checks payable to MOFGA Certification) if necessary. Because of the seasonal nature of this work and because we cannot issue a certificate for a crop after it is harvested, we cannot accept applications postmarked after June 1, 2026.
